Note：
(1) Material 1 After completing the form, print one copy, sign and seal the original and mail the original to the Secretariat.
(2) Materials 2, 3, 4 and 5 After completing the results, send the electronic version to the Secretariat: service@itma.org .
The association will confirm with the enterprise, and after confirming that they are correct, submit them to A&AC to start the authorized laboratory material review process.
Within three working days after the applicant's materials are reviewed by the association, the Secretariat will send the invoice of the authorized laboratory fee to the applicant, and the applicant shall complete the payment of the authorization fee within 30 days of receiving the invoice.
After the payment of the authorization fee, the ITMA official website publishes the information of the authorized laboratory unit, and arranges for the authorization plate to be issued to the applicant, and the application for the authorized product testing laboratory is completed.
The 3-year license fee for each laboratory is CNY 20,000 yuan.
The above are all RMB prices, other currencies should be calculated based on the exchange rate on the day the materials pass the review, the authorization fee needs to be paid in full, and the applicant needs to bear the tax and remittance fee.
